Interpretation - Notice in the last panel there is an exit door. From how Kaizou said "This feeling will never cease", he is desperately on the verge of giving up on his attempt to control his desire for humans' blood so the door... sort of symbolizes his desire to escape from this feeling. He didn't want to hurt Ahizuka because he's his friend and Kaizou is the kind of person that doesn't like to hurt people he loves. But the setting sun in the background shows that perhaps his hope of losing that feeling won't happen and when night falls, his desire becomes even greater. And he is afraid he might hurt Ahizuka.

That was long, wasn't it?

Ah, while I don't think the explanation was necessary, I agree the symbolism is very good. :3
